先天性脐静脉—门静脉系统发育异常新分类

胎儿脐静脉—门静脉系统(umbilical-portal venous system,UPVS)是指由双侧卵黄静脉和脐静脉发育而来的静脉系统,包括脐静脉、门静脉、静脉导管、肠系膜上静脉、脾静脉、肠系膜下静脉等血管。产前相对容易观察的血管主要有脐静脉、门静脉和静脉导管,其他血管产前评价极其少见。 UPVS在胎儿的血液循环中起着重要作用,脐静脉从胎盘输送含氧血,入肝连接左门静脉矢状部,末端形成静脉导管,将20%~30%含氧血直接汇入下腔静脉直达心脏,并经卵圆孔进入左心房,主要营养身体上部包括心脏、大脑等重要器官;肠系膜上静脉与脾静脉汇合形成门静脉,收集来自脾和胃肠的血液,到肝后与从左门静脉汇入的脐静脉血混合经肝进入肝静脉,再经下腔静脉入右心房,经右心室、肺动脉、动脉导管到达身体下半部分。随着近年来超声仪器分辨率的提高、多普勒技术及高分辨率彩色多普勒超声等的应用,对胎儿UPVS的观察及UPVS发育异常的病例报道越来越多,产前诊断准确率也不断提高。 |
