胎盘植入的产前超声新进展

1937年Irving首次对18例胎盘植入病例进行报道,并提出了“完全或部分异常粘连于宫壁”的组织学特征,这些“完整或部分蜕膜缺失”的指标沿用至今。20世纪60年代,病理学家根据子宫肌层内绒毛侵袭的深度,将胎盘植入分为3类:以胎盘绒毛单纯粘连在子宫肌层诊断为胎盘粘连(placenta creta,PC);以胎盘绒毛侵入子宫肌层诊断为胎盘植入(placenta increta,PI);以胎盘绒毛侵入子宫肌层并穿透子宫全层诊断为胎盘穿透(placenta percreta, PP)(图1)。此分类标准已沿用至今并为多数学者认可和使用。但在临床上区分胎盘植入的类别十分困难,尤其当胎盘植入、胎盘粘连和胎盘穿透同时存在于同一胎盘的情况下(图2),常在诊断时出现混淆,由于缺乏国际命名法共识,因此,将胎盘植入、胎盘粘连和胎盘穿透总称为胎盘植入谱,其中包括胎盘异常粘连和胎盘异常侵袭,或统称为异常侵入性胎盘(placenta accrete,PA)。 |
