精神分裂症患者任务正激活网络效应连接的初步探索
Altered effective connectivity in networks engaged during externally directed tasks in schizophrenia

文摘 目的探讨精神分裂症患者在工作记忆任务状态下和静息状态下,任务正激活网络(突显网络、中央执行网络及背侧注意网络)的效应连接特点.方法纳入42例精神分裂症患者及43名健康对照,运用基于频谱的动态因果模型分析方法(spectral dynamic causal modeling, spDCM),使用任务态下激活差异显著脑区提取突显网络、中央执行网络及背侧注意网络的感兴趣区种子点,分析上述脑网络在任务和静息态下的效应连接情况;并对存在差异的连接与临床症状及工作记忆表现进行多元线性回归分析.结果在静息状态下,患者背侧注意网络中的左侧额动眼区(后验参数=-0.25)及左侧顶上小叶(后验参数=-0.16)的自连接减弱,而突显网络中的前扣带回到左侧额动眼区兴奋性连接增强(后验参数=0.17);在任务状态下,患者左侧额动眼区自连接减弱(后验参数=-0.16),中央执行网络中的背外侧前额叶皮质(后验参数=0.20)自连接增强.相关分析结果显示,上述差异连接与健康对照的工作记忆成绩[R~2=0.22,F_(2,42)=5.84,P=0.006]以及患者的临床症状评分[R~2=0.19,F_(3,38)=3.12,P= 0.040]相关.结论不同状态下的任务正激活网络中,精神分裂症患者均存在异常效应连接.
其他语种文摘 Objective To explore effective connectivity of task positive networks (salience network(SN),central executive network(CEN) and dorsal attention network(DAN)) in patients with schizophrenia during working memory task and resting state. Method 42 patients with schizophrenia and 43 healthy controls were recruited and scanned by fMRI at resting state and under task. Spectral dynamic causal modeling was conducted to analyze effective connectivity of regions among the three networks under a working memory task and resting state separately.The regions within SN,CEN and DAN,shown significant differences between two groups, were selected as regions of interest within SN,CEN and DAN.Multiple linear regression analysis was employed to test effective connectivities, as well as the correlation to working memory performance and clinical symptoms. Results At resting state, patients with schizophrenia showed significantly less inhibitory self-connections in the left frontal eye field (LFEF) (posterior parameter=-0.25)(node in dorsal attention network)and in the left superior parietal lobe (LSPL) (posterior parameter=-0.16). The effective connectivity from the anterior cingulate cortex (node in salience network) to the LFEF was increased in patients(posterior parameter=0.17). Under the working memory task, patients with schizophrenia also showed decreased inhibitory self-connections in the LFEF(posterior parameter=-0.16), however increased self-connection in left dorsal lateral prefrontal cortex (LDLPFC)(node in central executive network) (posterior parameter=0.20).Moreover,the between-group connections difference was significantly correlated with the working memory performance in the healthy controls (R~2=0.22, F(2,42)=5.84,P=0.006), and correlated with the severity of clinical symptoms in patients with schizophrenia(R~2=0.19,F_(3,38)=3.12, P= 0.040). Conclusion The patients with schizophrenia may present with abnormal effective connectivity in task positive network both during task and at resting state.
